Cardiologists lay out the drill to prevent maternal cardiac arrest

Another young cardiac death has raised alarm bells about the basic status of women’s heart health. Popular Malayalam television actress Dr Priya died after suffering a sudden cardiac arrest while being eight months pregnant. And although doctors have managed to save the baby, questions are being asked as to the cardiac risks pregnant women have. Traditionally, female hormones were believed to protect the heart but now younger women are found to be exposed to certain stressors that make them just as vulnerable. And pregnancy, as we all know, puts an extra pressure on the heart and circulatory system.

Dr Priya succumbed to a sudden cardiac arrest, where the electric impulses in the heart malfunction and stop the heart beats. Therefore, all young women, including pregnant women need to undergo a basic heart check just like men. Says Dr Rajiv Bhagwat, interventional cardiologist, Nanavati Max Super Speciality Hospital, Mumbai , “Am sure all gynaeocologists advise this but one Echocardiogram during the cycle of pregnancy is needed to identify three things — Is the pumping function of the heart normal, if the valves are in a normal state and find out if there is any pre-existing condition that could pose a threat to the life of both mother and child.”

WHY WE NEED EMERGENCY MATERNAL CARDIAC CARE?

For the treatment of maternal cardiac arrest, an interdisciplinary team with expertise in both the maternal resuscitation procedure and the physiological changes that take place during pregnancy is necessary. “When maternal collapse unfortunately happens, resuscitative hysterotomy, often called a perimortem cesarean section, is a procedure that is frequently carried out during the second and third trimesters of pregnancy. Immediate hysterotomy may be necessary if the woman does not react to first cardiopulmonary resuscitation (CPR) and manual uterine displacement, says Dr Virendar Sarwal, Director, Department Of Cardiovascular and Thoracic Surgery, IVY Hospital, Mohali. Maternal collapse occurs in one in every 36,000 pregnancies, he adds.

EMBOLISM A RISK FACTOR

Sometimes, this could be a result of embolism too, where a clot from the leg veins or anywhere courses through the bloodstream and lodges itself in a heart artery blocking it. “This is a sudden development and depending on its massivity, a patient may not make it even when rushed to the hospital,” says Dr Bhagwat. Massive Pulmonary Embolism (PE) is an uncommon, but severe complication of pregnancy or during the first few weeks after giving birth, according to Dr Sarwal.

HOW PREECLAMPSIA IS A RISK FACTOR

Of course, pregnant women can have other risk factors, according to the American Heart Association journal. This could be chronic hypertension, gestational hypertension and preeclampsia, a condition that usually begins after 20 weeks of pregnancy in a woman whose blood pressure had been normal but rises steeply. Often manifesting as new-onset hypertension and protein in the urine during the third trimester, preeclampsia can progress rapidly to serious complications, including death of both mother and foetus. According to the European Society of Cardiology, studies have shown that women with pre-eclampsia were four times more likely to have a heart attack and three times more likely to have a stroke within 10 years of delivery than those without pre-eclampsia.

FAMILY HISTORY OF HEART DISEASE AND CARDIOMYOPATHY

Cardiomyopathy is a condition of the heart muscle where it stiffens up and makes it harder for the heart to pump blood to the rest of the body. It is usually genetic and can lead to arrhythmia or irregular heartbeats and ultimately heart failure. Dr Bhagwat once treated three sisters of a family who had inherited the condition and did not know about it. All three reported heart attacks in public places. “Once the condition is detected, one can put an implantable cardioverter-defibrillator (ICD) under the skin to stabilise heartbeats,” says he.

WHAT IS UNDEFINED CARDIOMYOPATHY?

Sometimes a heart attack could be the result of undefined cardiomyopathy, which is not caused by genes. This can lead to heart beat problems or arrhythmia, slow down the ability of the heart to pump effectively and accelerate clotting. “Sometimes long-term high blood pressure, obesity, diabetes and thyroid can create a stressor,” says Dr Bhagwat. He also points out how severe dehydration, when body salts go down, especially potassium and magnesium, can trigger very fatal rhythm disturbances and cause a heart attack.

WHAT ABOUT HEART VALVES?

The heart has four valves that keep blood flowing in the correct direction. Scarring or defects of the heart or its valves can raise the risk of complications during pregnancy. Compromised valve function means you might have trouble tolerating the increased blood flow that happens during pregnancy.

Two days after the shocking death of  actress Renjusha Menon,  Malayalam television actress Dr. Priya died on Wednesday after a cardiac arrest. The 35-year-old actor was eight months pregnant when she died at a private hospital. Notably, the actress had undergone a regular pregnancy check-up at the hospital recently before she suffered a heart attack. Her newborn baby is currently in the intensive care unit. 

Actor Kishor Satya broke the heartbreaking news to her fans on his social media accounts. ''One more unexpected death in the Malayalam television sector. Dr. Priya died of cardiac arrest yesterday. She was 8 months pregnant. The baby is in the ICU. There were no other health issues,'' he wrote.

''The mother who is crying unable to accept the death of her only daughter. The pain of husband Nanna as a loving partner with Priya without going anywhere for 6 months. Last night while going to the hospital, the sight of sadness rained in my mind. What will you say to comfort them? Why did God show this cruelty to those innocent minds who are believers?

The mind kept repeating the questions...Unanswered questions...Before the shocking news of Ranjusha's death goes away, next one more...When a person who is only 35 years old leaves this world, the mind is not allowed to say condolences....How will Priya's husband and mother recover from this collapse...Don't know...Let their minds have the power for that,'' the post added.

The 35-year-old actor was found hanging in the flat she shared with her husband Manoj, who is also an actor. Some reports say that she was going through financial troubles at the time of her death.

Pregnant Malayalam actress Priya dies of heart attack at 35

Popular Malayalam television actress Priya died on Wednesday after a heart attack. The 35-year-old was eight months pregnant and was undergoing regular pregnancy check-ups at a private hospital in Thiruvananthapuram in the southern Indian state of Kerala when she suffered a fatal heart attack, according to Indian media reports.

Her newborn is being cared for in the intensive care unit, the reports added. Priya survived by her mother, daughter, and husband Saravanan.

Actor Kishor Satya, her former colleague, shared the news of her demise to her fans through Instagram. As per Satya, Priya had no other health issues that could have led to the cardiac arrest.

In a heartfelt post, originally written in Malayalam, he said, “Her mother and husband are inconsolable. How do I comfort them? I don’t understand why they have to face such cruelty though they are God-fearing people. So many unanswered questions.”

The actor concluded the note, praying the family find comfort. “How can we comfort the family of someone who died so young? She was just 35 years old. Hope her mother and husband will have the strength to withstand this loss.”

Priya was a known figure in the Malayalam television industry. She gained recognition for her role in the romantic TV show Karuthamuthu. The show became the second longest-running television soap opera on Malayalam television. Kishor Sathya worked with Priya on the same show.

After getting married, Priya, who was also a doctor, took a sabbatical from acting. The Manorama website reported that Priya was pursuing an MD and was working at the PRS hospital in Thiruvananthapuram. She is survived by her mother, daughter, and husband Saravanan.

Priya’s death comes a day after the death of another . Menon, 34, was found dead at her residence in Thiruvananthapuram in the southern Indian state of Kerala, leading to speculation that she might have died by suicide. The Kerala police are investigating her death.

KERALA, India, Nov 1, (Agencies): Popular Malayalam television actress Priya passed away on Wednesday due to a heart attack. The 35-year-old, who was eight months pregnant, was undergoing routine pregnancy check-ups at a private hospital in Thiruvananthapuram, in the southern Indian state of Kerala when she suffered a fatal heart attack, as reported by Indian media.

Her newborn is currently receiving care in the intensive care unit. Priya is survived by her mother, daughter, and husband Saravanan.

Actor Kishor Satya, Priya’s former colleague, shared the news of her untimely demise with her fans on Instagram. According to Satya, Priya had no underlying health issues that could have led to the cardiac arrest.

In a heartfelt post originally composed in Malayalam, he expressed, “Her mother and husband are inconsolable. How can I console them? I cannot fathom why such cruelty has befallen them, considering they are devout individuals. Many questions remain unanswered.”

The actor concluded his note by offering prayers for the family’s comfort. “How can we provide solace to the family of someone who departed so young? She was just 35 years old. I hope her mother and husband find the strength to endure this loss.”

In a round of sad news coming from the Malayalam entertainment industry, TV actress Dr Priya passed away of cardiac arrest on Wednesday. The actress was 8 months pregnant and her newborn baby is currently in the NICU. Actor Kishor Satya informed fans of the sad news.

Priya was popular as a Malayalam television actress who was mostly known for her role in Karuthamuthu. She was a doctor by profession and took a break from acting after she tied the knot. As per reports, she was doing her MD and was working in Thiruvananthapuram’s PRS hospital.

Actor Kishor Satya shared the news on Instagram and wrote as per a report by NDTV, ”One more unexpected death in the Malayalam television sector. Dr Priya died of cardiac arrest yesterday. She was 8 months pregnant. The baby is in the ICU. There were no other health issues.”

The Malayalam actor further wrote that Dr Priya’s mom was inconsolable and that her husband’s pain cannot be written in words, “The mother who is crying unable to accept the death of her only daughter. The pain of husband Nanna as a loving partner with Priya without going anywhere for 6 months. Last night while going to the hospital, the sight of sadness rained in my mind.”

Kishor Satya further questioned God in his long note and referred to another death from the Malayalam entertainment industry – Renjusha Menon, who was recently found dead in her flat. The 35-year-old actor was found hanging in her flat.

The note further read, “What will you say to comfort them? Why did God show this cruelty to those innocent minds who are believers? The mind kept repeating the questions…Unanswered questions…Before the shocking news of Ranjusha’s death goes away, next one more…”

According to local reports, the actress had recently gone for a pregnancy check-up.
